We are excited to welcome our students back to school in person and begin the 2021-2022 school year at LACHSA!!
A copy of the instructions for the registration process is below. The same instructions are listed in the platform itself. Click the button to begin the process:
Instructions: This registration process contains forms required by LACHSA & LACOE. Most of the forms are required for the student to attend classes. You may have to create an account with "SurveyMonkey". Please click APPLY to begin. On the left side is a list of forms to download and complete. Please click on the name of the form. When you do, you will see instructions. Click on the title of the form to download it. All forms are downloadable and fillable PDFs. Then complete the form and upload to the corresponding field. The process could take up to 20-30 minutes in total. Helpful tips:
BE SURE TO CLICK "MARK AS COMPLETE" and all REQUIRED forms after you are done uploading your document (or completing a form).
optional forms do not need to be marked unless there was an upload.
Be sure to complete the forms listed as "optional" if they apply to your student. (If you uploaded a form, be sure to click "MARK AS COMPLETE")
Your task list is located on the left side of your screen.
When you are done, you will see a confirmation screen with more links to important information such as:
bell schedule and important dates
department meetings
department audition and placement meetings
school photo information
and more!
